This practice was established in 1965, and has been serving the people of this area continuously from this time. We see our role as helping you to be as healthy as you can be. To do this, we have a highly-committed practice team made up of different health professionals, working closely together. This allows us to better provide holistic healthcare, assisting you in staying well. Our aim is to prevent illness where possible, or achieve the best possible outcomes where it cannot.

We have built our practice around the model of the Patient-Centred Medical Home.

A patient-centred medical home is the general practice which is chosen by a person to be responsible for their ongoing, comprehensive, whole-person medical care. In a medical home, patients and their families have a continuing relationship with their GP and the general practice team. The whole team co-ordinates the care needed to optimise health. Mt Druitt Medical Centre has joined thousands of healthcare providers across Australia in registering in MyMedicare, a new voluntary registration system that will help us formalise the relationship between us and our patients.
MyMedicare allows you to nominate a general practice and general practitioner as your regular healthcare provider. When you register in MyMedicare, extra funding from the Australian Government becomes available to support our practice to provide more of the targeted care you need. 
Patients who register with our practice in MyMedicare may benefit from:
• longer MBS-funded telephone consultations, from 1 November 2023
• longer bulk billed telehealth consultations for children under 16 and Commonwealth card holders.
• connections to more appropriate care in general practice for people with chronic conditions who visit hospital frequently, from mid-2024.
Formalising the patient-practice relationship, like with MyMedicare, has been shown to improve health and wellbeing, and that is why we are inviting all of our regular patients to register in MyMedicare. 
If you have a valid Medicare card or Department of Veterans’ Affairs (DVA) Veteran card and have had two face-to-face consultations with us in the last 24 months, you are eligible for MyMedicare.
Registration is easy. To register as a MyMedicare patient, you can:
• fill out a paper registration form at our practice
• complete the registration process in your Medicare Online Account 
• ask our team to start the registration process – you can then complete registration in your Medicare

ESCALATE Project with the University of Sydney

We are participating in a 12 month trial of a new generic test aimed at helping to identify people who are generically prepositioned to heart disease with the University of Sydney. If you are interest in joining this free trail , please discuss this with your GP.

Dietary Advice

Our Dietitian and Nutritionist Dea Krismarietta sees patients for a range of dietary issues : Diabetes , Weight loss, Weight gain, Vegetarian diets, Heart health, Sports nutrition, Coeliac disease , Irritable Bowel Syndrome and Food allergies.

ECGs

An electrocardiogram (ECG) is a simple test that can be used to check your heart's rhythm and electrical activity. Sensors attached to the skin are used to detect the electrical signals produced by your heart each time it beats.

Spirometry

Spirometry is the most common of the pulmonary function tests. A spirometry measures the lung function specifically the amount and /or speed of air that can be inhaled or exhaled. Spirometry is helpful in assessing breathing patterns that identify conditions such as Asthma, Pulmonary Fibrosis , Cystic Fibrosis and COPD.

Minor Surgery

At Mount Druitt Medical Centre, our doctors are are able to perform simple surgical procedures under local anesthetic. Our team are trained an experienced in procedures such as skin cancer excisions and wound care.

Men's Health

Men's health includes: Prostate screening, Bladder problems, Pelvic pain ,Erectile dysfunction, Sexual problems and STD assessments, Cardiac assessments, General health checks, Preventative health screens, Bowel cancer screen, Blood pressure checks.

Women's Health

Women's health includes :Cervical screening, Sexual problems, Contraception, Pregnancy, Ovarian Cysts, Ovarian Cancer, Period problems and Menopause.

Cervical Screening Tests

The Cervical Screening Test changed on 1 December 2017 to improve early detection and save more lives. All women aged between 25 and 74 years are now invited to have a Cervical Screening Test every five years, instead of a Pap test every two years. The Cervical Screening Test detects HPV infection. HPV is a key risk factor in the development of cervical cancer. Even if you are vaccinated against HPV you need to participate in regular cervical screening.

Counselling

Carmen Debono , Master of Counselling (process) offers a warm and professional counselling experience for women. Practicing in Western Sydney and provides online counselling in the comfort of your home or office anywhere in Australia. Consultations are usually 55 minutes in duration at A$70. Carmen uses an integrated client-centered approach and provides counselling for the following: Anxiety, Stress management, Loss, Relationship challenges, Depression, Life transitions, Greif, Identity/ Self esteem and Goal setting.

Immunisations

The NSW Immunisation Program provides the community with protection against vaccine preventable diseases through initiatives targeting infants, adolescents, adults and at risk persons in line with the National Immunisation Program. This program is available through your GP.

Travel Health & Advice

If you travel outside Australia, you may get sick from a number of diseases that are preventable by vaccination. Different vaccines are needed for certain countries. You should consult your GP six to 12 weeks before you travel.

Antenatal Shared Care

Antenatal shared care (shared care) is joint care of a pregnant woman by her GP and the hospital antenatal clinic. Antenatal shared care creates the opportunity to practice collaborative obstetric care by combining the varied skills of each profession.

Diabetes Group Sessions

Your GP can refer you to either a diabetes educator, dietitian or exercise physiologist for an assessment. This involves taking a comprehensive history, identifying individual goals and preparing you for an appropriate group services program.

Chronic Disease Group Sessions

The care delivery team for SMAs is typically led by a general practitioner (GP) or advanced practice nurse. The team can include a group facilitator (eg. psychologist, nurse, diabetes educator) and sometimes a pharmacist, dietitian, exercise physiologist or other allied health professional (AHP).

Communication

Need assistance with communication? If you have difficulties with hearing or language, assistance with communication is available through the National Relay Service or the Phone Interpreters service. Please let our reception staff know if this is needed.

Urgent Clinically Significant Results

If your results are significantly abnormal or urgent, a doctor from this practice will contact you. Please make sure your contact details are up to date. You may also be able to discuss these results with your team by phone or video consultation

Privacy Policy

Purpose
This pamphlet explains how personal information about you and your health is recorded and managed in our practice. We also have a written privacy policy describing how we manage personal information. You can receive a copy of our policy free of charge upon request or access it via our practice website.
Personal information
The ‘personal information’ we collect includes your name, date of birth, address/es, contact details, Medicare number, healthcare identifiers and health fund details. Medical information may include medical history and any care you may need. GPs need information about your past and present health in order to provide you with high-quality care.
Our practice follows the guidelines of the RACGP’s Handbook for the management of health information in general practice, 3rd edition (the Handbook). The Handbook incorporates federal and state privacy legislation, and the Australian Privacy Principles, which requires that your personal information is kept private and secure.
Your medical records
This practice takes steps to ensure that your medical records:
  • are accurate, complete, well-organised and legible
  • are up-to-date
  • contain enough information to allow another GP to care for you
  • contain a summary of your care
  • can be used to remind you, with your permission, to return for follow up, check-ups and reviews.
If you are uncertain why information is being requested, please ask your GP or the practice staff.
If you wish to remain anonymous while accessing healthcare services, please talk to the practice staff.
 
Providing your information to other GPs
In this practice, it is normal for all GPs to have access to your medical records. If you have any concerns about this please discuss them with your GP or practice staff.
It is important that other people involved in your care, such as medical specialists and other healthcare professionals, are informed of the relevant parts of your medical history, so they can provide the best care for you. Your GP will let you know when this is necessary.
 
Providing your information to others
GPs respect your right to decide how your personal information is used or shared. For example, this may be sharing your health information with specialist doctors. Personal information that identifies you will only be sent to other people with your consent, unless there are exceptional circumstances. Gaining your consent is the guiding principle used by this practice in using and sharing your information.
Our practice will not share your personal health information with anyone else or another organisation unless:
  • you have consented to this sharing, or
  • they are legally obliged to disclose the information, in which case your GP will first discuss with you the information that she or he is legally obliged to disclose, or
  • the information is necessary for you to obtain Medicare payments or other health insurance rebates, or
  • there is an overriding public health and safety interest in the release of the information.
In the above cases, only information necessary to meet the requirements will be provided. Your health information will not ordinarily be sent overseas unless:
  • you are informed and provide consent for this to occur, and
  • the overseas country receiving the information has privacy laws that are very similar to the Australian Privacy Principles.
Using health information for quality improvement and research
This practice may use patient health information to assist in improving the quality of care we give to all our patients, by reviewing the treatments used in the practice.
Your information held by the practice may be used in research projects to improve healthcare in the community; however, this information will not include data that can identify you.
The information used for research, including the publication of research results, will not be in a form that would allow you to be identified, unless the research serves an important public interest. In such cases, identifiable medical records can be used for medical research without your consent under guidelines issued by the Australian Government. Before providing such identified information, your GP will discuss with you the information that she or he is obliged to disclose.
Security of information in the practice
Australian privacy legislation applies to all personal health information recorded in electronic and paper records. All records must be kept secure to protect against unauthorised access. This practice complies with these requirements to protect your information.
Yes, you do require an appointment. A standard appointment is ten minutes but if you require more time with your doctor you can request a longer appointment.
If you require an urgent consult, please call and we can arrange an appointment with your doctor. 
We bulk bill Health care card holders , Pension card holders and children 16 years and under for consultations in the practice. Telehealth consultations will incur a gap fee.

Our practice has policies in place for follow up of your results. A recall and reminder system is in place to ensure continuity of care. For further information, please discuss with your doctor or ask reception. 

When you book a telephone appointment , your doctor will call you at the time of the appointment or closest to the time as possible.
For a video appointment , you will receive a link to use at the time of your appointment. Please click on the link with a device that has a camera and speaker, follow the prompts and wait for the doctor to also log into the video.
Should you require a translator for your consultation, please advise reception , so they can arrange this service for yo. For further information please ask our friendly reception staff. 
We understand that your time is valuable and all doctors work hard to keep on time but there are times when doctors may run late due to emergencies or unforeseen circumstances we encourage patients to phone and see how their doctor is running prior to coming in if waiting might be a problematic, or if you live close to the clinic and it may be more convenient to wait at home.
